Assistance to Individuals in Crisis Situation may cover medical, burial, transportation, education, food, or other crisis support after a social worker’s assessment.
- Prepare a valid ID and documents matching the need.
- For medical help, prepare a recent medical certificate or abstract plus the bill, prescription, or laboratory request.
- A barangay certificate or social case study may be required depending on the case and amount.

02PCSO Medical Assistance
The Medical Assistance Program uses an online application for eligible medical requests and can issue a Guarantee Letter or Claim Slip when approved.
- Create an account only on the official Online MAP website.
- Upload the required ID and medical documents.
- Monitor the registered email or mobile number for status.
- If approved, print the documents and follow the instructions for the partner health facility.

Use a clear subject such as “Medical assistance request: patient name, province.” In the message, include the patient’s full name, age, location, contact number, assistance needed, short situation summary, list of attached documents, and preferred safe contact method. Do not send passwords or one-time PINs.

Gumamit ng malinaw na subject gaya ng “Medical assistance request: pangalan ng pasyente, probinsya.” Isulat ang buong pangalan, edad, lokasyon, contact number, tulong na kailangan, maikling paliwanag, listahan ng attachments, at ligtas na paraan ng pagkontak. Huwag magpadala ng password o OTP.

Check spelling, dates, doctor details, signatures, license numbers, and document validity. Keep the originals and a copy of every submission. Save the reference number, confirmation email, or screenshot.
